2) Upload a Headshot and a picture of the front of your OSHA Card.

3) Upload a picture of EITHER your current SST card or BOTH the 8-Hour Fall Prevention AND 2-Hour
Drug and Alcohol Awareness Certificates.
4) Pick up your card at: 147 Prince Street, Brooklyn, NY 11201. If you were referred by someone, include
the name in the form. Your card will be ready within 2 business days.
