2) Upload a Headshot and a picture of either 30-hour OSHA Card OR 10-Hour OSHA Card, 8 Hour Fall Prevention Certificate, and 2 Hour Drug and Alcohol Awareness Certificate.

3) Choose a delivery method. We can either mail your card to the address you provided. Or, you can pick up your card at: 147 Prince Street, Brooklyn, NY 11201. If you were referred by someone, include the name in the form.
